At approximately 6:19 p.m., the Polk County Sheriff’s Office and Polk County Fire Rescue responded to reports of a serious crash at the intersection of North Church Avenue (State Road 37) and NE 5th Street. According to investigators, Jeremiah was driving a white 1999 Jeep Grand Cherokee northbound on North Church Avenue when the vehicle struck a curb. The impact caused the Jeep to rotate and flip multiple times. Jeremiah, who was not wearing a seatbelt, was ejected during the rollover. First responders found him lying at the corner of the intersection with severe injuries. He was rushed by ambulance to a nearby hospital, where he was later pronounced deceased.

The roadway remained closed for nearly three hours as emergency personnel worked tirelessly and investigators processed the scene. The investigation into the crash remains ongoing.
