VERMILLION TOWNSHIP, Ohio — The community is mourning the tragic loss of two lives following a devastating three-vehicle crash that occurred early Friday morning, February 13, 2026, at the intersection of U.S. Route 30 and State Route 511.

Malachi James Harley John Whitehead, 23, of Wooster, and David L. Kuhns, 61, also of Wooster, were both pronounced deceased at the scene after a collision that involved a 2015 Dodge Dart, a 2024 Peterbilt semi-truck, and a 2020 Ford Transit van. The Ohio State Highway Patrol Ashland Post is actively investigating the circumstances of the accident.

According to reports, the crash occurred at approximately 6:00 a.m. when the Dodge Dart, driven by Malachi Whitehead, failed to stop at a stop sign on SR 511 and was struck by a Peterbilt semi-truck traveling eastbound on U.S. 30. The force of the impact caused the semi-truck to cross the median and collide with the Ford Transit van traveling westbound, ultimately leaving both vehicles off the north side of the highway.

Malachi Whitehead, a young man known for his vibrant personality and close ties to the Wooster community, tragically lost his life at the scene. Those who knew him remember him as full of energy, kindness, and always ready to lend a helping hand to friends and family. His life, though brief, left a lasting impression on everyone who had the privilege of knowing him.

David L. Kuhns, a respected member of the Wooster community, was the driver of the Ford Transit van and also tragically passed away at the scene. He is remembered for his dedication to his family and friends, his warm spirit, and the countless ways he contributed to his community over the years. David’s loss leaves a deep void for his loved ones and everyone whose lives he touched.

The driver of the semi-truck, Mac A. Johnston, 24, of South Pasadena, Florida, was transported to UH Samaritan Hospital with non-life-threatening injuries. A juvenile passenger in the Ford Transit van was life-flighted to OhioHealth Mansfield Hospital with serious injuries. Officials have confirmed that all occupants involved were wearing safety belts at the time of the crash and that alcohol or drugs are not suspected as contributing factors.

The crash response involved a coordinated effort from multiple agencies, including the Ashland County Sheriff’s Office, Ohio Department of Transportation, Ashland County EMA, Ashland Fire & EMS, Hayesville Fire & EMS, Life-Flight, the Ashland County Coroner’s Office, and Aber’s Towing.

As the community grieves, thoughts and prayers go out to the families of Malachi Whitehead and David Kuhns, as well as to those injured in this tragic accident. Memorials and tributes are pouring in, reflecting the profound impact both men had on those around them.
